RODO - Information obligation

Based on Article. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Regulation on data protection), OJ EU.L.2016.119.1 (hereinafter: GDPR), we kindly inform you that:

The administrator of the personal data obtained is MARKMED Rehabilitacja Ruchowa Marek Wiecheć, ul. Kopernika 14, 27-400 Ostrowiec Świętokrzyski. 1. Your data is processed for the purposes of:

 a) Article 6(1) 1 letter c and art. 9 section 2 letter h GDPR in connection with joke. 25 point 1 of the Act on Patient Rights and the Patient Ombudsman of November 6, 2008 and Art. 10 section 1 point 2 of the Regulation of the Minister of Health of November 9, 2015 on the types, scope and templates of medical documentation and the method of its processing in order to establish identity before providing the service, in particular by registering for medical care, verifying data when arranging remote visits as well as in the facility at reception desks or in the doctor's office.

b) Article 3(1) 1 of the Act on Medical Activities of April 15, 2011 for the purpose of providing health services.

 c) Article 9(1) 2 letter h GDPR in connection with joke. 24 section 1 of the Act on Patient Rights and the Patient Ombudsman of November 6, 2008 and the Regulation of the Minister of Health of November 9, 2015 on the types, scope and templates of medical documentation and the method of its processing for the purpose of maintaining and storing medical records.

 d) Article 6(1) 1 letter c GDPR in connection with joke. 9 section 3 and art. 26 section 1 of the Act on Patients' Rights and the Patient Ombudsman of November 6, 2008 and Art. 8 section 1 of the Regulation of the Minister of Health of November 9, 2015 on the types, scope and templates of medical documentation and the method of its processing for the purpose of receiving and archiving authorization to access medical documentation and providing information on health status.

 e) Article 6(1) 1 letter f GDPR in order to implement the legitimate interests of the Administrator, in particular by telephone to confirm or cancel a medical visit.

 f) Article 6(1) 1 letter c GDPR in connection with joke. 74 section 2 of the Act of 29 September 1994 on Accounting in order to process accounting and tax documentation, including: keeping accounting books, issuing invoices for services provided, which may involve the need to process personal data.

Who do we share your data with?

 In order to maintain the continuity of processes, we may transfer your data for processing to entities with which we have appropriate cooperation agreements. Especially:

a) entities processing data on our behalf, participating in the performance of our activities:

b) operating our IT systems or providing us with IT tools

c) entities providing us with advisory, consulting and auditing services as well as legal, tax and accounting assistance, and research agencies acting on our behalf.

d) other data controllers processing data on their own behalf under a data entrustment agreement. e) authorities authorized under the law to receive your personal data.

You have the right to:

• access to the content of your data - by exercising this right, you have the opportunity to obtain information about what data is processed, how and for what purpose,

• rectification of your data - by exercising this right, you can report to us the need to correct incorrect data or supplement data resulting from an error in collecting or processing data,

• Providing personal data is obligatory under the law, and otherwise voluntary.

• limiting the processing of your data - using this right, you can submit a request to limit data processing if you question the correctness of the processed data. If the request is justified, we can only store the data,

• deletion of your data - using this right you can submit a request to delete your data. If the request is justified, we will immediately delete the data. However, this right does not apply to situations where personal data are processed for purposes related to fulfilling the legal obligations of the controller or to perform a task carried out in the public interest or within the public authority entrusted to the controller,

• raise an objection - by exercising this right, you can at any time object to the processing of your data if they are processed pursuant to Art. 6 section 1 letter e or f (legitimate interest or public interest). After accepting a request in this matter, we are obliged to stop processing data for this purpose. In such a situation, after considering your request, we will no longer be able to process personal data subject to objection on this basis, unless we demonstrate that there are valid legally justified grounds for processing data that are considered by law to override your interests. , rights and freedoms or grounds for establishing, pursuing or defending claims,

• submit a complaint to the supervisory authority, i.e. the President of the Personal Data Protection Office, when it is found that the processing of your personal data violates the la
